To make an appointment, contact our office at 818-902-2800, Monday to Friday from 9 AM - 5 PM. If you are a new patient, please have your insurance card ready. If you are a return patient and have had a change in insurance, you will need to update your insurance information.

For new consultations, please print and fill out completely the new patient questionnaire (form).  This will expedite the check-in process and reduce your wait time.

We do our best to expedite appointments for urgent injuries.

If you have outside X-rays or imaging, please bring a copy of the CD and report(s). Also, if you have pertinent outside doctors’ notes or other medical information, please bring a copy of those as well. 

If you need to cancel your appointment, please give us 48-hour notice. This will open up the time slot for other patients.

Due to COVID-19 precautions, we are asking patients to come alone to the appointment, or have the family member/friend wait outside the waiting room.
